I Give You My First Love (2009) follows the poignant journey of Takuma, a young boy diagnosed with a severe heart condition that necessitates regular care from a cardiologist. During his hospital visits, Takuma forms a deep bond with Mayu, the doctor's young daughter. One fateful day, Mayu overhears her father discussing Takuma's prognosis, revealing that his condition is grave and he is unlikely to survive past the age of 20. Unaware of this grim prediction, Takuma, at just 8 years old, makes a heartfelt promise to marry Mayu when they both turn 20. As they grow older, Takuma becomes painfully aware of his impending fate and begins to distance himself from Mayu, hoping to spare her the heartache of losing him.
